Uyo Model Search entry forms are free. Entry Forms have been sponsored by Big Exude N Cakes, master bakers and sugarcraft confectioners. Entry Closes October 11, 2013. Forms are downloadable online @ www.uyomodelsearch.com and www.wetinhappen.com. Forms can also be picked, completed and returned at OTIS Model Academy (IMAGE FACULTY), 129 Ikot Ekpene Road, Uyo. Uyo Model Search will be held in the city of Uyo, Akwa Ibom State – Nigeria, and is open to Akwa Ibom girls only;
1. Aged 16 - 20 Years old
3. With Striking Looks
4. Height, standing at 5.9ft (1.79m) or Above
5. Maximum hip size not more than 37 inches (93cm)
6. With Good Stage Projection
7. With or Without Catwalk or Runway Skills

ENTRY RULES

I am between the ages of 16 and 20 (as of 31st December 2013).

If any of the information I have given in the entry form is found to be false, Kardynal will be entitled to exclude me from the Model Search contestant selection process.

I grant to Kardynal irrevocable performers release and state that Kardynal may photograph, film, record, edit, broadcast, rebroadcast, publish or communicate to the public my performance, including for promotional purposes, related to the Uyo Model Search without remuneration.

If I am selected as finalist by Kardynal, I agree to authorize Kardynal to conduct background checks in order to verify any of the information I have supplied.

If I am selected, I agree I am able to travel as an individual or with a chaperone as appointed by Kardynal.

I am in possession of a valid passport or identity document and understand and agree that if I am selected as finalist in the Uyo Model Search, I will be required to produce my valid passport or identity document at the audition.

I agree to make myself available to interviews, auditions, publicity, and any other event relating to the Uyo Model Search 2013.

I warrant that I am psychologically, mentally and Physically fit to relate and work with other contestants and the Kardynal throughout the selection process and possibly the boot camp of this competition.

I agree that the 15 finalist that will be selected to appear in the final live fashion spectacular will be judged by the official(s) chosen by Kardynal and those of the International agents collaborating with Kardynal.

I Warrant that I understand and accept these Entry/Competition Rules, that I have no legal obligations which conflict with the making and signing of this Application in accordance with the Entry/Competition Rules.
